WHILE STACKING A BAG ON A PALLET ON A FLAT-BED TRAILER, EMPLOYEE SLIPPED ON A SLICK SPOT AND FELL OFF THE DECK, RECIEVING A LACERATION TO THERIGHT CHEEK, A GASH ABOVE THE RIGHT EYE AND A TWISTED SMALL FINGER ON HIS RIGHT HAND.
