AN UNINTENTIONAL ROOF FALL ON HAZY SECTION, ENTRY #5, TRAPPED THE RIPPER HEAD TO THE MINE FLOOR. THE FALL WAS 12'L X 16'WIDE AND 4'THICK. THE ROOF SUPPORT WAS 4'RESIN W/6' TORQUE TENSION NO INJURED NO PASSAGE BLOCK.
