EMPLOYEE WAS DRIVING STEEL WEDGE INTO SEEM OF STEEL SHAFT FORM DOORS. THE WEDGE BOUNCED OUT WITH THE HAMMER BLOW. WEDGE STRUCK EMPLOYEE UNDER RIGHT EYE ON HIS CHEEK BONE.
