EMPLOYEE WAS DRILLING A HOLE WITH A PIECE OF DRILL STEEL WITH A BOLT WRENCH ON THE BOTTOM. THE WRENCH SLIPPED OFF STEEL STRIKING EMPLOYEE ON THE HEAD.
