TWO EMPLOYEES WERE INSTALLING A BELT SPEED REDUCER. ONE STEPPED DOWN AND HIS FOOT BECAME ENTANGLED INBETWEEN 2 TURN BUCKLES CAUSING HIM TO FALL.
