ON 4-23-02 EE COMPLAINED OF NECK AND SHOULDER PAIN WHICH STARTED ON 4-16-02, HE THOUGHT IT WOULD GO AWAY, BUT IT GOT WORSE. SENT HIM TO DOC ON 4-23-02. EE WAS SACKING AT THE PACK HOUSE.
