Narrative

EE STATED THAT HE WAS INSTALLING A ROOF BOLT IN NO.1 ENTRY AND HE TRIPPED AND FELL TWISTING HIS LEFT KNEE. THE LOCATION IN WHICH THE INCIDENT HAPPENED WAS WET AND ON A DOWNWARD GRADE. EE BEGAN MISSING TIME ON 5/3/02.
