A METAL ROOF MAT OR STRAP HAD BEEN DISLODGED ON ONE END EVIDENTALY BY THE CONTINUOUS MINER OR SHUTTLE CAR. THE STRAP WAS HANGING DOWN FROM FOOR AND THE EMPLOYEE WALKED INTO IT.
