EE HAD INSTALLED TWO ROWS OF BOLTS IN BREAK FROM #4 TO #3 ENTRIES WITHOUT INCIDENT ON SIX MILE (013MMU). AFTER T-BAR HAS BEEN SET, HE WAS POSITIONING THE HEAD TO DRILL RIB BOLT WHEN A PIECE OF ROCK HIT HIM IN THE FOREHEAD.
