Narrative

AN UNINTENTIONAL ROOF FALL OCCURRED ON THE 9TH WEST HEADGATE UNIT #2 ENTRY AT 3425 WEST. THE FALL WAS APPROX 40' LONG, APPROX 18 FT WIDE AND APPROX 12-15 FT THICK. THE FALL DID NOT AFFECT V ENTILATION OR PASSAGE OF MINERS. THE FALL WILL BE CLEANED UP.
