Narrative HE WAS REMOVING A TILT JACK ON A D-11 DOZER. HE HAD ONE FOOT ON THE PUSH ARM AND PLACED THE OTHER FOOT ON THE TRACK. THE TRACK WAS MUDDY AND HISFOOT SLIPPED CAUSING HIM TO FALL ON THE TRACK.
