Narrative

EE WAS FOLDING 6 FT CONVEYOR BELT USING CHANNEL LOKS. THE BELT WAS WET CAUSING CHANNEL LOKS TO SLIP. EE LOST FOOTING FALLING TO GROUND - LANDING ON RESCUER AND LIGHT. EE INJURED RIGHT WRIST AN D LOWER BACK.
