EMPLOYEE WAS ATTEMPTING TO REMOVE SLING FROM A HOOK. EMPLOYEES WERE LOWERING THE DRAG CHAIN DOWN THE SIDE OF THE BERM. EE STARTED DOWN THE SIDE OF THE BERM WHEN HE TRIPPED AND FELL.
