EMPLOYEE WAS LOADING TRACK TIES INTO A SCOOP BUCKET. A PIECE OF ROOF COAL 2-4 IN THICK, 2-4 FT WIDE FELL FROM BETWEEN BOLTS AND STRUCK EMPLOYEE ON HEAD AND SHOULDER. EMPLOYEE ATTEMPTED TO JUMP OUT OF THE WAY AND TWISTED HIS RIGHT ANKLE.
