EE WENT TO STEP OVER BERM AND STEP ON ICE AND SLIPPED AND FELL- HE TOLD HIS SUPERVISOR THAT HE WAS OK ONLY HIS PRIDE WAS HURT. SUPERVISOR WAS MADE AWARE OF THE SLIP AND FALL ON THE SAME DAY- HUMAN RESOURCES WAS NOT INFORMED UNTIL 4-08-03
