Narrative EMPLOYEE INDICATED THAT AT THE BEGINNING OF THE SHIFT HIS RIGHT ARM FROM ELBOW TO HAND WAS SORE FROM OPERATING TRACTOR EQUIPMENT. HE WAS DIAGNOSED WITH MEDIAL EPICONYLITIS & ULNAR NEUROPATHY. HE HAD SURGERY ON HIS RIGHT ELBOW ON 5/6/03.
