Narrative

EE WAS BLOWING 2"" SHOTCRETE HOSE TO CLEAR IT PRIOR TO SHOOTING MATERIAL. A PLUG OF MATERIAL TEMPORARILY BLOCKED HOSE THEN SUDDENLY EXPELLED CAUSING EE TO LOSE GRIP ON NOZZLE & KNOCKING EE TO G ROUND.
