THE EE WAS SHOVELING ACCUMULATIONS FROM THE WALKWAY ONTO THE MAIN NORTH BELT AS HE WAS SHOVELINGTHE SHOVEL HIT THE MINE ROOF THEN STRUCK THE (TOP) MOVING BELT PUSHING THE SHOVEL INTO HIS CHEST AND TWISTING HIS RIGHT SHOULDER.
