Narrative

THE EMPLOYEE WAS SINKING AN ANCHOR BOLT INTO THE FLOOR WITH A JACK HAMMER. THE HAMMER SLIPPED OFF THE BOLT CAUSING THE EMPLOYEE TO OVEREXTEND HIS RIGHT ARM/SHOULDER.
