EMPLOYEE WAS WALKING FROM END OF 22ND WEST TRACK TO SECTION, STEPPED IN A SMALL HOLE IN THE MINEFLOOR, FELL JAMMING AND TWISTING HIS LEFT KNEE. HIS LAST DAY OF WORK WAS 10-27-03, KNEE SURGERY ON 10-28-03.
