Narrative

EMPLOYEE WAS ROOF BOLTING AND HAD INSTALLED A ROOF BOLT AND WAS IN THE PROCESS OF LOWERING THE BOOM. AS THIS WAS BEING DONE, HE TURNED & SLIPPED WITH HIS KNEE GOING UNDER THE BOOM.
