## Narrative

Individual was working on a 10 foot tall, single section scaffold while positioning, cutting and welding metal wear plates in crusher discharge surge bin.   Individual stepped off scaffold and fell 10 feet to the ground.   Individual was not wearing his provided fall protection nor was there a handrail section installed on the scaffold.
