EE was backfilling bore holes with a shovel & complained of lower back pain and numbness in his right leg.EE was treated as a first aid case. 10-27-03, he was taken Ctr for Occ Hlth & examined & was diagnosed with a lumbar strain. Given Rx & released to return to work to operate a drill.10-30-03, he was re-evaluated and Physical Therapy was set up. for surgery on 11-21-03.
