Narrative

INJURED WAS GETTING INTO 486 SCOOP AND SLIPPED ON OIL IN THE OPERATOR'S COMPARTMENT. THIS WAS A FIRST AID INJURY UNTIL 12-3-03. INJURED CONTINUED TO WORK UNTIL 12-3-03, AT WHICH TIME HE BEGAN LOSING TIME FOR ROTATOR CUFF SURGERY.
