Narrative

EMPLOYEE WAS HOLDING A PIN KNOCKER AGAINST THE PIN ON THE BUCKET TOOTH OF 994 ENDLOADER. AS ANOTHER EMPLOYEE STRUCK THE PIN KNOCKER WITH A SLEDGE HAMMER, A PIECE OF METAL CAME OFF PIN KNOCKER AND HIT EMPLOYEE ON LEFT THUMB CAUSING A LACERATION THAT REQUIRED 5 STITCHES.
