EE STATES WHILE HELPING REMOVE BELT STRUCTURE, HE LIFTED UP ON AN H-BEAM WHILE REMOVING CHAIN AND HE FELT HIS SHOULDER PULL. THIS DID NOT BECOME A LOST TIME ACCIDENT UNTIL 12/30/03, AS EE HAS WORKED HIS NORMAL JOB SINCE ACCIDENT DATE.
