Narrative

EE said he feels like the incident occurred while he was shoveling coal at the loading station at the mine. He does not remember a specific incident that caused his left elbow & left index finger to start hurting. On 12/31/03 a Dr. diagnosed tendonitis in his left elbow & left triger finger caused by repetitive trauma from shoveling. Placed on restricted duty 12/31/2003
