Narrative

EMPLOYEE WAS STANDING BESIDE THE ROOF BOLTER MAKING UP BOLTS. HE HAD HIS FOOT IN FRONT OF THE TIRE WHEN THE OPERATOR PULLED BOLTER UP TO PUT IN ANOTHER RUN OF BOLTS AND RAN OVER HIS FOOT.
