JUSTIN WAS SHOVELING FINE, CRUSHED ROCK WHEN HIS BACK BEGAN TO HURT BUT DID NOT REPORT TO SUPERVISOR. HE SAW HIS WON DOCTOR ON 2/8/04. REPORTED TO OUR CLIENT ON 2/10. GEOTEMPS RECEIVED CLIENT' S INCIDENT REPORT ON 2/17. ALCOHOL & DRUG SCREEN SCHEDULED FOR 2/18/04.
