EMPLOYEE WAS TRYING TO TIGHTEN A LARGE VALVE AND HE STEPPED ON LARGE PIPE WRENCH. HIS FOOT SLIPPED OFF THE WRENCH AND HIS ANKLE HIT A ROCK ON THE GROUND.
