Narrative

ON 1-25-04 THE MINER WAS HOLDING ON TO CHAINS ATTACHED TO A CRANE. ONE OF THE CHAINS GOT HUNG UP ON A CHUTE AND WHEN IT RELEASED IT STRUCK HIM ON THE HARD HAT. BECAME AWARE OF PHYSICAL THERAPY ON 3-19-04.
