## Narrative

Employee claims that while repairing the cyclone air lock on top of hydrate bin #5, he placed a section of 2"" X 8"" wood across a guard rail support and a metal frame  about 14 inches from the floor.  The employee than stood on top of the 2X 8 to assist is repairing the air lock.  The wood broke and he fell to the floor, striking his right arm above the wrist.
