Narrative

Employee was in the process of drilling a hole to install a roof bolt. He did not get the drill steel correctly installed in the drill chuck and when he turned on the rotation the drill steel struck his right hand resulting in a fracture to the hand. He was treated by a doctor on 3/18/04.
