Narrative

EE climbing step ladder, he'd leaned against a portion of a dust collector in order to repair a portion of ducting. as he was reaching to secure his fall protection, the ladder feet kicked out. He fell about 3 ft to the top of conveyor hood & then to walkway 4 ft below that. he recvd scalp laceration, & xray revealed distal shoulder fracture.
