## Narrative

Jackleg fell off a deck as it was being transported. The employee lifted the drill back on the deck and experienced pain in his right lower back and first aid treatment was given. Late in March the employee requested medical evaluation for continuing pain. He  had a microdiskectomey surgery on April 2, 2004 for a herniated nucleus pulposus repair at L5 - S1.
