## Narrative

While adjusting the packing on tank 29 D.o.v., the wrench slipped off the nut hitting employee  in the mouth chipping his two front teeth.  He did not need any immediate medical attention, but has an appointment with the dentist on 4/15/04 to repair his chipped teeth.
