Narrative

Employee was attaching new support cables to A frame poles on a maintenance truck working off an eight foot rolling step ladder. While attaching the cables to the A frame pole location the cable slipped out of the clamps at the other end of the cable allowing the A frame poles to fall pushing over the ladder.
