Ee complained of pain in his right hip and leg from no particular incident. Ee is a haul truck driver. He was taken to the doctor, diagnosed with tendonitits and was released to full duty. However, the condition continued to worsen and surgery was performed on 3/11/04 resulting in the incident transitioning in to a reportable/lost time.
