According to most recent audiogram ee has suffered a recordable hearing loss in both ears. This ee may be exposed to noise above 85 decibels (A-scale) for an 8-hr time weighted average while on the job. Hearing conservation training & hearing protection has been provided to ee upon hire & annually thereafter. In addition, ee's supervisor has been trained in hearing conservation.
