## Narrative

Area was well lighted, a scaling bar was within easy reach of miner. The miner did not properly bar down the area before approaching the face, miner reached up to re-align bolt on belt drive when loose piece of rock dropped off from area and slid down rib, hitting left shoulder and arm.
