Narrative

The employee was wrenching on a bolt, when the wrench slipped and struck him in the mouth, chipping his tooth. He saw the dentist on April 26, 2004 and tooth #8 was filled. The dentist indicated that at the time no root canal or cap was necessary. The employee was released to full duty.
