Narrative

Employee was using a 24"" pipe wrench to remove a broken stud from the front wheel of an End Dump when he experienced pain in his right shoulder. He was examined by a doctor on 4/30/04 and diagnosed with tendinitis in his right shoulder and placed on restrictive duty.
