Narrative

Employee strained his back when a copper cathode sheet slipped out of his hands. He was given a full duty release on 03/04/04. He was still feeling pain the next day and went back to the clinic and was told to take the weekend off. Employee was scheduled to work on Saturday. The Safety Dept. was not aware of the second visit or that he had missed a day of work.
