EE WAS CLEANING HIS AREA AT THE START OF SHIFT AND SLIPPED ON THE ICE.  ON 5/5/04 EE HAD SURGERY ON HIS KNEE AND TURNED THE INJURY INTO A LOST-TIME ACCIDENT.
