## Narrative

The employee was using a sledge hammer to drive keeper pins from dragline teeth, and experienced pain in his right shoulder.  At the time no medical attention was sought.  On May 12, the employee reported that he had been receiving medical treatment, and needed surgery on his shoulder.  He missed his first scheduled shift on May 14, 2004
