Narrative

Employee was changing the shim packs in a coupling and had to cut the bolts off. He was driving out the bolts when a sliver broke off and hit his right shoulder. May 24, 2004, he went to doctor to get sliver removed from his shoulder. Reclassified 5/25/04 from FA to MT due to medical treatment on 5/24/04.
