Narrative

On May 21, 2004 the employee reported pain in his left elbow and arm. The employee is a mechanic and the pain apparently is caused by years of repetitive motion repairing heavy equipment. The employee has been evaluated by an orthopedic and is currently receiving physical therapy. To date no time has been lost as a result of this incident.
