Narrative

The employee was working on D11R dozer at a height of 11'1"" while standing on a step located on the left side of the dozer cab. The employee was removing a chain which was supporting a left ripper tilt cylinder. The step broke and the employee fell to the ground causing a compound fracture below the knee to the right leg.
