## Narrative

Employee was lifting pallets for majority of shift.  At 4:00 pm he experienced pain in right hip, lower abdomen and lower back.  On May 19th the employee had difficult time getting out of bed, he called in sick.  He went to the doctor on 5/19 and again on 5/26 and was diagnosed with a hernia.  He has a doctor's appointment on June 2nd.
