Narrative

Injured was struck by a piece of roof rock measuring 83"" long x 31"" to 13"" wide x 8"" to 9"" thick. The rock fell from between the rib and bolts without warning. Roof conditions appeared to all be good in the area. Bolt spacing was in compliance.
